What are system-wide incident investigations?
In 2017, the Healthcare Safety Investigation Branch (HSIB) was founded to develop a system-based approach to safety investigations in healthcare. The purpose of this was to improve the ways that investigations identify methods of prevention for similar incidents rather attributing blame and liability on individuals.
A system-wide incident investigation examines the systems in place as a whole to identify flaws in the system that supports the delivery of care rather than the individuals involved in an incident. These types of investigations are effective in identifying areas of improvement within a service as a whole rather than attributing blame for errors to an involved party.

Why system-wide investigations are beneficial
Healthcare services are a made up of complex systems of people, teams, processes, technology, environments, and equipment. A system-wide approach to investigations allows us to assess the complex interactions between all elements of the delivery of care and avoid a linear approach to discerning the cause of an incident.
This allows us to avoid placing unnecessary blame or fault on an individual in cases where the care delivery system as a whole was the major factor that resulted in an incident occurring. A system-wide incident investigation may identify individual accountability, but this only happens when the system itself is not a major contributing factor.
How do our investigations work?
Delphi utilises a range of investigative approaches that show the potential causes of a specific event and a review and analysis of patients’ clinical notes, risk assessments and care plans can identify areas of risk and suggest actions for improvement.
We also utilise a PSRIF template to discover the causes of a problem and its solutions; alongside a detailed timeline of events. Due to their impartiality, Delphi are able to provide an unbiased and evidence-based, factual report based on the evidence provided.
All investigations are peer reviewed for accuracy, consistency and proportionality. Delphi use open and closed questions in interviews with staff involved in an incident, with a ‘no blame’ philosophy.

Our composition of staff includes Occupational Therapist, RMN, RGN, LD specialists, CQC Inspectors, Operational Managers, all with many years of experience at senior level both within the Public and Private sectors.
