How do our investigations work?
Our investigations team utilise a range of investigative approaches to identify the potential causes of a specific event. We also review and analyse patients’ clinical notes, risk assessments and care plans to uncover areas of risk and suggest actions for improvement.
Alongside a detailed timeline of events, we also utilise an in-depth PSIRF template to discover the causes of a problem and its solutions. Impartiality is at the core of our service, which allows us to provide an unbiased and evidence-backed report, based on the evidence provided.
All investigations completed by our team are peer reviewed to fully ensure accuracy, consistency and proportionality. Additionally, we follow a ‘no blame’ philosophy throughout our investigations. This philosophy flows all the way down to even the most basic aspects of our service.

Kate is a highly qualified investigations lead with a law degree specialising in medical law and neuroscience, providing her with a deep understanding of the legal and ethical complexities in healthcare. With over six years of experience as a CQC inspector, Kate has developed a strong expertise in managing serious incidents within healthcare and social care settings.
She is proficient in utilising Datix and other leading systems for incident reporting and management, ensuring a meticulous and thorough investigative process. As an investigations lead, Kate excels in conducting independent, system-wide analyses using the SEIPS tool, applying the principles of human factors and ergonomics to uncover root causes and provide actionable recommendations.
Kate's ability to lead independent investigations for your trust or social care service ensures that patient safety is prioritised, and high standards of care are maintained. Her insights and expertise will help your organisation learn from incidents and continuously improve the quality of care delivered.
